The Warm Handoff provides services to individuals and families who are going through complicated illnesses and/or the dying process manage the complexities of the medical system, community services, coordinating care, schedules and considerations for end of life. Illness can come with a myriad of emotions and obligations. I have a decade of experience in healthcare in many settings, that provide me a unique view into healthcare navigation. The services I provide are non-medical and complement programs such as Hospice and Care Coordination. 

What is a Warm Handoff?

A warm handoff is a term used between medical providers to transition care to another provider or service, with the involvement of the patient and their care team so they can ask questions or clarify their needs. 
The Warm Handoff is my purpose. I can advocate for you as part of your personal care team, ensuring communication and understanding between yourself and your family, medical providers and other resources. I am here to accompany you on this journey, making no judgements nor any decisions.
